ABCD State Resources

North Carolina developed a best practice comprehensive community model that builds on its "physician driven" enhanced primary care case management program, Community Care of North Carolina, and is characterized by introduction and integration of a standardized validated screening tool and collaboration with local and state agency staff and families. Since the ABCD project officially ended, the stakeholder group has continued to meet, policy improvements have been adopted at the state level, and additional practices have replicated the ABCD model.
